When individuals receive proactive coordination, person-centered support, from an interdisciplinary approach, while utilizing evidence-informed practices, individuals experience improved outcomes, communities become safer and healthier, and public resources are used more effectively.
PAC-NW will achieve this by:
- Providing integrated, interdisciplinary direct services in the community;
- Measuring and evaluating program outcomes
- Developing innovative models of care; and
- Advocating for sustainable policy improvements throughout the spectrum of care

Our programming is still in development as we consult with experts and external stakeholders. It will be modeled and inspired from Maslow’s Hierarchy of Needs, Forensic ACT services, Housing First, comprehensive risk assessment, ORS/OAR requirements and other evidence based practices.
